This presentation discursively takes the issue of the delayed implementation process of the integrated diabetes education program of the St. Paul Regional Health Center (SPRHC). The main ones are the absence of communication and integrated care that harms the interaction between professionals and allows not to involve patients in using self-management strategies. Being the NURS FPX 4005 assessment 4 focus, the proposed solution implies the use of an interdisciplinary approach to treatment with the following objectives in mind: the betterment of seasonal diabetes management outcomes, the enhancement of communication, and the strengthening of patient education.
Organizational Issue and Significance
Among the largest organizational problems at SPRHC is the fact that the program of complete diabetes education needs to be implemented, which has not been established yet. This lag leads to poor clinical results and a low rate of adherence. All these factors lead to inefficient processes in division of labor, lack of proper working standards, and the variances in the level of communication amongst the providers all of which are present in this stakeholder presentation.
The result of these gaps is misaligned treatment plans and a lack of glycemic control, increase in hospital readmissions, and eventually healthcare costs. Employee morale is one more aspect affected by the care coordination issue and increases the risk of burnout and employee turnover. The significance of NURS FPX 4005 assessment 4 is also emphasized by the fact that the inadequate care of diabetic
patients may negatively affect the image of the hospital by discouraging participation and recruiting novice employees.
Tandan et al. The recommended strategy will improve glycemic control, reduce hospital readmission rates, and boost patient adherence with the use of technology and evidence-based strategies. For SPRHC to achieve its mission of offering comprehensive, patient-centered diabetes care, strong cooperation, resource allocation, and leadership are required.
