This presentation discursively takes the issue of the delayed implementation process of the integrated diabetes education program of the St. Paul Regional Health Center (SPRHC). The main ones are the absence of communication and integrated care that harms the interaction between professionals and allows not to involve patients in using self-management strategies. Being the NURS FPX 4005 assessment 4 focus, the proposed solution implies the use of an interdisciplinary approach to treatment with the following objectives in mind: the betterment of seasonal diabetes management outcomes, the enhancement of communication, and the strengthening of patient education.

Organizational Issue and Significance
